Dundee care home fined £20,000 over resident’s cleaning fluid death

A Dundee care home has been fined £20,000 over the death of a resident who drank cleaning fluid.

An inquest at Dundee Sheriff Court found David Fyfe died from ammonia poisoning after drinking cleaning fluid that was left in an unlabelled bottle in his room at Tigh-Na-Muirn in Monifieth in 2020.

The court heard the cleaning fluid had been left on top of a bathroom cabinet in the room of the resident who was living with Alzheimer’s, the BBC reported.

Sheriff Jillian Martin-Brown said: “Paramedics transferred Mr Fyfe to hospital, where he gradually deteriorated until his death.

“The primary cause of death was acute tracheobronchitis and pneumonia, resulting from the ingestion of ammonium-based cleaning product.

“It is not known why the bottle of Steri-Germ had no label. With the benefit of hindsight, Steri-Germ ought not to have been stored in an accessible place in Mr Fyfe’s room.”

Fyfe died four dies after drinking the fluid at Ninewell Hospital in Dundee on 31 May 2020.

A spokesperson for the home said: “We would like to offer our sincere condolences to the family.

“We have learned from this tragedy and taken all the necessary steps to ensure this never happens again.”
